The form captures all details to go on the customised cards, plus parent information and the option to pay online or via direct deposit.

There are two package options;
- a pack of 5 cards (all the same card) or a pack of 10 cards.
If there's interest from an entire team to purchase 'full-team sets' which will include one card of each team member, this can be arranged via discussion with Michael Farnell on 0431 967 496.
