Medical Form

Some medical conditions may have an impact on a player's ability to participate or that participation could affect the condition.  In these circumstances, the club needs to identify this at sign on and the coach and manager be informed if specific medical assistance could be necessary.

For common conditions players may have such as asthma, epilepsy and diabetes, the club, coaches and magers need to be informed.  For players with these conditions, an emergency management plan needs to be developed with the parents.

Players with more significant conditions such as heart problems or who have lost one of a paired organ system (e.g., kidney) need a specific medical clearance to play.

A Medical Form for all players will be kept in the managers folder. For players with any medical conditions an emergency management plans or a medical clearance should also be attached to the form.
