Member protection form
The Member Protection Declaration is issued by Basketball Victoria and needs to be completed by all coaches and committee members over the age of 18. This is in addition to the Working with Children Card.
The Member Protection Delaration must be witnessed by one of the professionals on the list below. The completed form must be returned to the committee where it will be stored securely. These forms must be completed every 2.5 years while a coach is with the Wildcats.
