BA acknowledges and respects your privacy. Information collected is for the purposes of commencing or renewing membership and associated functions, including promoting services or opportunities which may be of interest to you. Information provided by you is collected, used and disclosed in accordance with our privacy policy (available on the BA Website). Terms and Conditions of participation
To be agreed to by the participant or person listed as Parent / Guardian 1 if the participant is under 18 years of age.
I authorise and consent to the applicant undertaking the BA Activities. In consideration of the applicant's participation in the Activities, or membership with BA and any other BA Organisation being accepted, I expressly agree to be responsible for the applicant's behaviour and agree to accept the terms set out in this membership declaration. In addition, I agree that the applicant and I are bound by, and shall comply with, the BA Constitution and any by-laws and policies made under it.
I have read, understood, acknowledge and agree to the above including the exclusion of implied terms, warning, assumption of risk, release and indemnity.
Basketball Queensland
1. In this declaration: "The Association" means the association which you are registering with; "Member Organisation" has the same meaning as in the BA constitution and where the context so permits, the Member Organisation in which you apply to become a member; "BA" means Basketball Australia Limited; "BQ" means Basketball Queensland, "Basketball Activities" means performing or participating in any capacity in any authorised or recognised Basketball Organisation activity; "Basketball Organisation" means and includes BA, BQ and The Association and the affiliates and where the context so permits, their respective directors, officers, members, servants or agents.
2. If my application for membership of The Association is accepted I will be bound by and agree to comply with the constitutions, regulations and policies of Basketball Australia, Basketball Queensland and The Association.
3. Fitness to Participate: I declare that I am medically and physically fit and able to participate in the Basketball Activities. I will immediately notify the Association of any change to my medical condition, fitness or ability to participate. I understand and accept that the Basketball Organisations will continue to rely upon this declaration as evidence of my fitness and ability to participate.
4. Medical Treatment: I consent to receiving any medical treatment that The Association reasonably considers necessary or desirable for me during my participation in Basketball Activities. I also agree to reimburse The Association for any costs or expenses incurred in providing me with medical treatment.
5. Right to Use Image: I acknowledge and consent to photographs and electronic images being taken of me during my participation in any Basketball Activities. I acknowledge and agree that such photographs and electronic images may be used by The Association or BQ for promotional or other purposes without my further consent being necessary.
6. Privacy: I understand that the information I have provided above is necessary for the objects of the Basketball Organisations. I acknowledge and agree that the information will be disclosed to BQ and BA and will only be used for the objects of the Basketball Organisations, Basketball Organisation general business and to provide me with membership services. I understand that I will be able to access the information through my Member Organisation. If the information is not provided my membership application may be rejected. I acknowledge that the Basketball Organisations may also use my personal information for the purposes of providing me with promotional material from Basketball Organisation sponsors or third parties. I may advise my Member Organisation if I do not wish to receive from the Basketball organisations, any sponsor or third party promotional material. I understand that I can access the Privacy Policy of BQ for further information about this.
