STEP 1:- 

• At the Stadium the player (or carer if U18) completes an Injury Report Form & asks the Court Controller or Referee Supervisor to sign it.
• Hand the form to the canteen and ask for a copy for your records. 
• On the next office day, the Stadium Manager will email the injury form to BNSW on your behalf and record in the injury register.
